2021 Church Membership Application

We invite your church to be a member of the Pine Haven Christian Assembly ministry. We believe Pine Haven has a vital ministry among the Christian Churches/ Churches of Christ in Minnesota and we hope you will support us in providing quality programs that proclaim God’s word and call young and old alike into a relationship with Jesus Christ.

Member Benefits
• Member Churches are considered part owners of the camp
• Can nominate someone from your congregation for board elections
• Are allowed one ministerial vote and two lay vote at the annual meeting
Member Responsibilities
• $30.00 annual membership fee and application


To submit your church membership application for 2021, please fill out the form below OR download and fill out the Church Membership Application Form. Please send your form and/or membership fee to Pine Haven Christian Assembly, 16508 State Hwy 87, Park Rapids, MN 56470.

Please note:  Receipt of both the membership form (online or mailed in) and a donation of at least $30.00 before January 1, 2021 are required for 2021 membership.
